In September 2011, the Rockbridge Area Free Clinic received a federal grant from the US Health Resources and Services Administration, Bureau of Primary Health Care, to conduct a year-long planning initiative to prepare for a possible application to become a Federally Qualified Health Center (FQHC). The Rockbridge Area Free Clinic partnered with Carilion Stonewall Jackson Hospital, the Health Department, and other community partners to conduct a complete community health needs assessment (CHNA). The findings clearly demonstrate the need for increased access to basic health services and an expanded safety net across all assessment activities. Active community engagement in the CHNA process resulted in the successful transition of the Rockbridge Area Free Clinic to a FQHC, thus increasing affordable access to health care services in the region.
Currently, community leaders are in the process of conducting the 2015 CHNA in Lexington City, Buena Vista City, and Rockbridge County. Active participation in this process is extremely vital to ensure we continue to address the health of our community and create solutions that ultimately result in a better place for our citizens to live.
We need you help! The online community health survey deadline has been extended until June 5th. The survey can be accessed online at www.surveymonkey.com/s/2015CHNA. Please encourage community members to complete the survey!
